Platelet Therapy in Veterinary Medicine

This week, we have a special guest blog about platelet therapy use in veterinary medicine from Dr. Amber Vibert. Dr. Vibert is VetStem’s Safety and Technical Services Veterinarian and has extensive experience in both general and emergency veterinary medicine.

I’m very excited to have the opportunity to contribute to our blog today! As VetStem’s new clinical veterinarian, I’m here to give you an added layer of information from a medical perspective. Today I’d like to share with you the capabilities of wonderous cells called platelets. You may have heard the term “Platelet Rich Plasma” (PRP) or “Platelet Enhancement Therapy” (PET) and wondered, “What are platelets and how does this treatment work, exactly?” We have showcased several success stories of pets who have received platelet therapy and now it’s time to look at the science behind the medicine and applications for which they can be used.

Good Things Come in Small Packages

Platelets are very small cells found in the blood stream of mammals and are best known for their ability to clot the blood. However, there is SO MUCH MORE these tiny but powerful cells can do! A complex signaling system sent out from damaged cells attracts platelets to an injury and tells them to release several healing molecules that they have stored inside of them. In turn, these healing molecules attract a multitude of additional healing cells (including stem cells) to the site of injury or inflammation. Together, these cells have been shown to reduce pain, remove the damaged cells, build new blood vessels, prevent further tissue damage, and generate new healthy cells in place of the injured ones. Amazing!

What Can These Heroes of Healing Be Used For?

In veterinary medicine, platelet therapy is most often used for treatment of joint-related problems such as cruciate ligament tears, osteoarthritis and tendon injuries in dogs, cats, and horses. However, recent research has shown that PRP/PET can also aid in the healing of skin wounds, corneal (eye) ulcers, surgical incision sites, tooth sockets following extraction, and even muscle tears. And the use of platelet therapy is not just limited to our animal companions. You may have heard of NFL players who have received PRP/PET for tendon/ligament injuries and muscle tears. Platelet therapy can also be used in conjunction with stem cell therapy to maximize the effect of both treatments.

Harnessing the Power of Platelets

The functions of platelets may be complex, but their collection and administration is quite simple. A calculated amount of blood is drawn based on the patient’s size/weight. The blood sample is then either spun in a machine called a centrifuge or injected through a special filter such as VetStem’s V-PET™ gravitational filter system in order to separate the platelets from other blood cells. The final product is a highly concentrated number of platelets suspended in the protein-rich fluid component of the blood called plasma. This solution is then injected (or topically applied as with skin wounds or surgical incisions) to the injury site. And voila! There you have platelet therapy- another way to enhance the body’s own power to heal.

Injured Veteran’s Dog Receives Stem Cells and Platelet Therapy

This week is International Assistance Dog Week (IADW). Information from the IADW website states, “International Assistance Dog Week was created to recognize all the devoted, hardworking assistance dogs helping individuals mitigate their disability related limitations.” To show our support of this well-deserved recognition, we wanted to highlight Max, a service dog to a disabled army veteran. According to an article from Florida Today, Max has been a trained companion for U.S. Army Staff Sergeant Edward Johnson since 2014. Sgt. Johnson, a purple heart recipient, was shot in the head during combat in Iraq in 2006 and was left with a traumatic brain injury. Max helps Sgt. Johnson cope with PTSD and other debilitating ailments related to his injuries.

In 2017, Max was diagnosed with a torn cruciate ligament. He was obviously in pain and in need of surgery and other medical procedures. Fortunately, his story got out and through donations and good will, Max was able to have surgery. His surgeon, Dr. Jeff Christiansen of Superior Veterinary Surgical Solutions, donated his services and organized donations from several others as well. As an experienced VetStem provider, Dr. Christiansen recommended stem cell and platelet therapy in conjunction with the surgery to aid Max’s healing. VetStem provided a free Veterinary Platelet Enhancement Therapy kit as well as discounted stem cell processing services.

According to Dr. Christiansen, Max recovered completely. Unfortunately, Max suffered a second cruciate rupture in his other leg just over two years after the initial surgery. Once again, Dr. Christiansen and several companies, including VetStem, stepped up to provide this dog with top-notch care. Max received surgery on his other knee in addition to stem cells and platelet therapy. In this video from Dr. Christiansen, Max can be seen working on his at home exercises with his dad.

Bunji Received Surgery and Platelet Therapy for Broken Forelimb

We recently received news that one of VetStem’s most experienced users, Dr. Jeff Christiansen, treated an extra special patient with surgery and Platelet Rich Plasma using a kit that was donated by VetStem.  But what makes this patient so special, you ask?  Well, it was a Kangaroo! 

Bunji is a 2-year-old Red Kangaroo who lives at the Brevard Zoo in Melbourne, Florida.  In July during the daily morning check-in, his keepers found him injured and while it was clear he had broken bones, there was no known cause of the injury.  X-rays revealed he had fractured his radius and ulna in his right forelimb and surgery was indicated to ensure proper healing.

Dr. Christiansen, a veterinary surgical specialist, was called in for the job and performed the surgery on Bunji.  A plate was placed to hold the bones together.  He also used Bunji’s own bone marrow to aid in healing the fracture site.  Lastly, he treated Bunji with Platelet Rich Plasma utilizing the Genesis CS-2 kit, which is typically used to treat horses. 

First, Dr. Christiansen collected blood from Bunji, which he then centrifuged to concentrate the platelets.  Once the platelets were concentrated and collected, he injected the platelet rich plasma directly into Bunji’s surgical site.  Platelets, when exposed to damaged tissue, release signals to attract healing cells to both speed up and increase the quality of healing.

Bunji spent approximately two months in recovery at the zoo animal clinic before he was returned to the Kangaroo Walkabout.  He is reported to be doing well and seems happy to be reunited with his Kangaroo buddies.  Though we have not done any work with a Kangaroo before, Bunji is not the first exotic animal that VetStem veterinarians have treated.  In April, we reported that a Sun Bear at the San Diego Zoo received VetStem Cell Therapy for his arthritis.

Veterinarian Highlight: Dr. Nick Vitale, DVM

For this week’s Veterinarian Highlight, we would like to introduce you to Dr. Nick Vitale of Heritage Animal Hospital in Dundee, MI.  Dr. Vitale received his BS in Zoology and his DVM from Michigan State University.  Though a relatively recent adopter of VetStem Cell Therapy, Dr. Vitale hit the ground running and has treated over 40 patients with stem cell therapy in less than a year and half.  And that doesn’t include the patients he has treated with Veterinary Platelet Enhancement Therapy (V-PET™)!

Though stem cells are primarily used to treat orthopedic conditions such as osteoarthritis and injured tendons or ligaments, Dr. Vitale has treated his patients for several “non-standard” indications as well including feline kidney disease and Inflammatory Bowel Disease.

We spoke with Dr. Vitale to ask a few questions about how he uses regenerative medicine in his practice.

Why do you like stem cell therapy and what do you commonly use it for?

I like it for many reasons. I can help reduce a patient’s need for medicines that have bad secondary side effects. I can help multiple issues at once, vs an owner having to perform multiple orthopedic surgeries which can be very, very expensive when the owner only has so much to work with.

Ideally, I would love to perform stem cell therapy on each orthopedic patient I take care of from ACL injuries to OCD issues to medial coronoid disease, etc. If my own lab needed ACL surgery, I would automatically perform stem cells to help the surgery even more. My normal surgical discussions, especially for ACL injuries, is to discuss standard of care surgery such as MRIT or TPLO and then I say, ‘we can make it even better with doing this.’ I also bring up that when we bank these doses, we can use them later in life for other problems such as autoimmune disease or liver or kidney problems, as this is most likely going to be a treatment very commonly done in the future. 

What injuries/ailments do you commonly treat with Veterinary Platelet Enhancement Therapy (V-PET™)?

I use V-PET for chronic non-healing injuries and for use on ligamentous and tendon injury. For very early ACL strains and tears, less than 25%, I have performed V-PET and have had good outcomes so far. I also will do this for a last resort when a patient has a less than 50% tear of an ACL and there are no other options as far as surgery, which is too expensive for the owner. Older patients, especially those with cancers, I will do V-PET on for arthritis vs doing stem cells. In very, very old patients, V-PET is easy to do and apply and it will keep them mobile longer and I have seen very good results with this modality.

Great Dane Receives Relief from Platelet Therapy

In January, we reported about a Great Dane that was suffering from an injury to her cruciate ligament.  Pinky, who is seven years old and approximately 170 pounds, slipped on a wet driveway and tore her right rear cruciate ligament.  She was non-weight bearing on her injured leg and was unable to go on her daily walks and struggled with her normal activities such as getting in and out of the car and going up the stairs.

Pinky’s owner, Rebecca, was initially told surgery was Pinky’s only option.  Rebecca decided to seek a second opinion with Dr. Douglas Stramel of Advanced Care Veterinary Services.  Dr. Stramel is a Certified Veterinary Pain Practitioner and offers advanced pain management techniques including VetStem Regenerative Cell Therapy and Veterinary Platelet Enhancement Therapy (V-PET™), which he recommended to aid Pinky’s ailing knee.

After a simple blood collection, Pinky’s blood was run through the V-PET™ system to create an injectable platelet concentrate rich in natural healing cells.  The concentrate was injected into Pinky’s injured knee and the healing cells began their work.

We recently checked in with Dr. Stramel and Rebecca to see how Pinky is doing.  Pinky is over 8 months out since her last treatment with platelet therapy and according to her mom she is doing great!  She walks a total of about 1.5-2 miles per day and Rebecca stated, “she is full of energy and gets very excited when I ask if she wants to go for a walk.  At night when she sees the motion detection lights in the backyard, she runs out the door to go chase opossums.  She has no trouble going up and down stairs and does so easily.”  While Pinky continues to take a nonsteroidal anti-inflammatory, Dr. Stramel pointed out that she takes a dose that is about 75mg lower than a typical dose for a dog her size.  Way to go Pinky!

Platelet Therapy: A Complement to Stem Cell Therapy

In addition to stem cell processing services, VetStem distributes platelet therapy kits to small and large animal veterinarians across the United States and Canada.  Platelet therapy is similar to stem cell therapy in that the patient’s own cells are collected, concentrated, and then reinjected into the affected area.  Unlike stem cell therapy, platelet therapy requires a blood collection and the process of concentrating the healing cells is performed by your veterinarian in the clinic.

How does platelet therapy work?  The scientific answer is that platelets activate by exposure to damaged tissue, releasing their granular contents which include anabolic growth factors.  These growth factors help attract progenitor cells to the injury site and play a key role in stimulating tissue repair through fibroblast expansion and cellular matrix production.  In other, less technical terms, when the concentrated platelets are injected into the site of damaged tissue, the platelets signal additional healing cells to migrate to the affected area to begin the process of tissue repair.

The great thing about platelet therapy is it can be performed in conjunction with stem cell therapy to further aid the healing process.  In our opinion, stem cell and platelet therapies are very different regenerative medicine solutions that can work synergistically. They each have their place and can benefit patients in different circumstances. We see the combination of adipose stem cell therapy and platelet therapy as the “platinum standard” for regenerative medicine.  While the idea of stem cell therapy is to deliver as many regenerative cells to the affected area as possible, by adding platelet therapy on top of it, additional healing cells will migrate to the area to further stimulate local tissue repair processes.  And like stem cell therapy, platelet therapy is autologous, meaning the animal is both the donor and the recipient.  Thus, there is minimal risk of rejection and reaction when performed under sterile conditions.

Our primary platelet therapy product for small animals is Pall Veterinary Platelet Enhancement Therapy or V-PET™.  We’ve seen much success with V-PET™ such as in Pippa Rose’s case and Pearl’s case.  But, similar to stem cell therapy, every patient’s response will vary.  Your veterinarian can best determine if your dog may benefit from platelet therapy.

Pit Bull with a Chronic Wound Treated with Platelet Therapy

Pearl is a pit bull who is missing about one quarter of her right front leg.  She was found as a stray and veterinarians were unable to determine why part of her leg was missing.  Pearl’s owner Julia had a prosthetic leg made for Pearl when she was young.  It helped her to run, swim, fetch balls, jump and play with other dogs.

After a while however, Pearl began to develop a wound at the end of her stump that made wearing her prosthetic uncomfortable.  Julia sought treatment for the wound, however nothing seemed to help and she was faced with the possibility of amputating the remainder of Pearl’s right front leg.

Julia had a surgical consult with Dr. Holly Mullen of VCA Emergency Animal Hospital and Referral Center in San Diego, California to discuss amputation and also address a partially torn cruciate ligament in Pearl’s right rear knee.  While surgery could fix both issues, Dr. Mullen suggested trying platelet therapy first.

Utilizing the Pall Veterinary Platelet Enhancement Therapy system (V-PET™), Dr. Mullen treated Pearl’s chronic, non-healing wound and also her partially torn cruciate ligament.  Julia stated, “Within two weeks of the platelet therapy treatment you could see a tremendous increase with the weight she was putting on her right rear leg and by four weeks her stump had completely healed.”  Julia was very grateful for Dr. Mullen’s recommendation to try platelet therapy before jumping into a big surgery.  It was this treatment that saved Pearl’s right front limb from being completely amputated.
